Leros to Tilos is a southbound leg on Dodekanisos Seaways' Dodekanisos Express, taking around two and a half hours. The catamaran calls at islands including Kalymnos, Kos and Nisyros on its way down the chain. It is a fast craft service, so the crossing is quick and mostly indoors.

Ferries dock at Livadia, the main village on Tilos and where nearly all the island's rooms and tavernas are. Tilos is one of the least developed islands in the Dodecanese, known for hiking trails, birdlife and quiet beaches, and it has taken its conservation credentials seriously for years. The Charkadio cave, with its fossilised dwarf elephant remains, is the island's best-known curiosity.

The direct sailing runs about once a week, so it suits a fixed plan rather than a spontaneous one. Current departures and fares are listed below. Tilos is more regularly connected to Rhodes and Kos, and routing through either is usually easier if the dates do not line up.
