Fourni to Tilos runs a little over eight hours on Blue Star Ferries' Blue Star Patmos, heading the length of the Dodecanese chain with island calls on the way. It is a conventional ferry, so there is room to move about, a bar and open decks for the crossing. This is a day's travel rather than a hop.

Ferries dock at Livadia, the main village on Tilos and the island's principal settlement. Tilos is among the least developed islands in the Dodecanese, quietly eco-conscious and known for hiking trails, birdwatching and empty beaches. Inland is the older settlement of Megalo Chorio, along with the Charkadio cave and its fossilised dwarf-elephant remains.

Sailings run roughly once a week, so dates need checking early. The current timetable and fares appear in the table below. Rhodes and Kos both have more regular connections to Tilos as an alternative.
