Mytilini to Leros is one of the longest single legs on the Blue Star Ferries eastern Aegean line, taking between roughly twelve and thirteen and a half hours on the Diagoras. The ship calls at Chios, Samos and Patmos on the way south, which accounts for the spread in journey times. It is a conventional car ferry with cabins, so an overnight sailing is manageable.

The ferry port on Leros is Lakki, a deep natural harbour lined with 1930s Italian Rationalist buildings that give arrivals an unusual look for the Aegean. Lakki is separate from Platanos, the island's older main town, and from the beach village of Agia Marina, both a short drive away. Leros is quiet and low key, with the hilltop Castle of Panteli above Platanos and little in the way of package tourism.

The route runs a couple of times a week, so the sailing date is the main constraint. Current schedules and fares are shown below. From Lakki there are frequent onward links within the Dodecanese to Kalymnos, Kos and Rhodes, plus local boats to Patmos and Lipsi.
