Mytilini to Patmos is a long southbound leg of the Blue Star Ferries line, worked by the Diagoras, with calls at Chios and Samos on the way. Sailing times run from roughly ten to around thirteen hours depending on the routing, so this is an overnight crossing in practice. Cabins are available and worth taking on a run this length.
Patmos is a Dodecanese island whose port and main settlement is Skala, with the hilltop Chora above it. The fortified Monastery of St John the Theologian crowns the ridge above the harbour, and the Cave of the Apocalypse sits on the road between the port and the Chora. The historic centre is a UNESCO World Heritage Site, and development on the island has stayed unusually restrained as a result.
Sailings run a couple of times a week, so this is a route to plan around. The current timetable and fares are shown below. From Skala there are regular onward connections through the Dodecanese to Leros, Kalymnos, Kos and Rhodes.
