Patmos to Astypalea crosses west out of the main Dodecanese chain toward the island that sits closer to the Cyclades in both position and character. Dodekanisos Seaways runs the leg with the Dodekanisos Pride, and it takes around four hours and ten minutes. It is one of the longer runs the fast craft makes, and because Astypalea is off to one side of the usual north-south route, direct sailings are uncommon.

The ferry docks at Skala, also called Pera Gialos, immediately below the Chora. The Venetian kastro crowning the hill above the harbour is the island's signature sight and visible as you come in. Astypalea is whitewashed, quiet and popular with Greek visitors rather than package tourism, with easy beaches and Cycladic-style lanes in the Chora. There is also a small airport for domestic flights, useful if the boats do not fit your dates.

Direct sailings run only a couple of times a fortnight, so this is a route to book once you have found a date that works rather than turn up for. The live timetable and fares are shown below. Travellers who cannot make the direct leg usually route via Kalymnos or Kos, both of which have more regular Astypalea connections.
