Patmos to Megisti, better known as Kastellorizo, is the full run to the far southeastern corner of Greece, around thirteen and a half hours on the Blue Star Patmos. The ship calls at several Dodecanese islands before making the long final leg past Rhodes. It is a genuine sea journey and usually done overnight.

Kastellorizo is Greece's easternmost inhabited island, sitting just off the Turkish coast near Kas. The port town wraps around a small bay in a horseshoe of pastel neoclassical houses, which is the image most people know from the film Mediterraneo. There is little to do beyond eating well by the water and taking a boat out to the Blue Cave, and that is rather the point.

The route runs about once a week and only during the season. Current departures and fares are shown in the table below. The island also has a small airport with flights via Rhodes, which is the quicker way in or out.
