Tilos to Agia Marina on Leros is a catamaran run with Dodekanisos Seaways aboard the Dodekanisos Express, taking around two and a half hours. It is a fast craft with mostly enclosed seating, calling at other Dodecanese islands as it works north. Expect a brisk, functional crossing rather than a leisurely one.

Agia Marina is one of the two main ports on Leros and the commercial and administrative heart of the island, with a working harbour feel. Italian rationalist buildings from the 1912 to 1943 administration stand near the waterfront, including the clocktower and the market. Leros is quieter than nearby Kos or Patmos, with calm beaches and a good deal of wartime and Italian-era history to poke around.

The direct sailing runs about once a week, so it needs planning into a route rather than treating as an everyday option. The current timetable and fares are shown below. Leros's other port, Lakki, a few kilometres away, handles the larger conventional ferries and gives more options for onward travel to Piraeus and the northern Dodecanese.
