Tilos to Agios Kirykos on Ikaria is one of the long cross-Aegean legs that Blue Star Ferries links together on its Dodecanese to North Aegean run, worked by the Blue Star Patmos. The crossing takes around eight and three quarter hours, calling at other islands as it works its way north. It is a full-size conventional ferry, so there is a restaurant, indoor lounges and deck seating for what is a proper day at sea.

Agios Kirykos is the capital and main port of Ikaria, a workaday harbour town rather than a polished resort. The town's thermal spa facilities sit near the harbour, part of Ikaria's long hot-springs tradition, and the island is better known for its unhurried pace and its reputation as a Blue Zone for longevity than for sightseeing. Note that Ikaria has a second port at Evdilos on the north coast, used by some routes instead of Agios Kirykos.

This is a once-a-week sort of connection rather than something you can turn up for, so it needs planning into an itinerary rather than treating as a fallback. The current timetable and fares are shown below. If the direct sailing does not fit, the usual alternative is to work north through Patmos or Samos and pick up a separate boat from there.
